University of Northern Colorado Health Care Survey

The University of Northern Colorado is conducting an extensive study of health care and insurance issues among our students. If you were randomly selected to participate in this survey, please note that the deadline for participation has been extended to noon on Monday December 11.

Please contact our survey manager, Elise Wainwright  (click here for email), at 970-351-1916 if you have questions about the survey or have difficulty accessing the survey using the last six digits of your student identification number.

We expect the survey will take you approximately 10 to 15 minutes to complete. The survey includes questions about your use of health care providers and recent health care conditions that required treatment. You may preview the survey by using the Student Identification number: 123456.  The only inquiries that you are required to answer are as follows: 

Demographic questions 1 through 13 pertaining to sex, age, ethnicity, country of citizenship, etc.,
Questions 19 and 32 (number of visits to the Student Health Center and number of sessions at the Counseling Center) 
Question 50 ("What is the source of your health insurance coverage?"). 
Questions 19, 32, and 50 are required because the set of questions you will receive following each inquiry will be different depending upon your answer (e.g., you will be sent to different questions if you have zero visits to the Student Health Service in the past 12 months versus an answer indicating you had one or more visits to the Student Health Center).  Other than the questions noted above, you may leave all other questions blank.  When you click on the "submit" button at the bottom of each page, the survey software will call your attention to any questions you did not answer.  You may then use the "back" button on your web browser to answer the question, or click on a "continue" button to bypass the question(s) you choose not to answer.  If you inadvertently answer a question incorrectly, there is a button at the bottom of each page of the survey that allows you to clear all answers on that page. 

Confidentiality and Your Options as a Survey Participant

We are doing everything possible to assure the confidentiality of the survey participants' responses.  Although we have taken appropriate steps relative to survey data confidentiality, such as using a secure server, there is no way to provide an absolute assurance that data transmitted over the Internet will remain secure.   The partial use of your student identification number to access the survey is only used to reduce the possibility that students will take the survey multiple times and to select the students for the participation prize drawings.   Once you enter the last six digits of your ID number, our survey program assigns a random number to the data string line for your answers.  Thus, UNC only knows if you have accessed the survey.  We do not know if you have completed the survey after you have accessed the survey by entering the last six digits of your student identification number.

You may quit the survey anytime prior to moving to the comments section on page 12 (i.e., answering question the last question on page 11 clicking on "Okay, on to page 12") by either closing your web browser or hitting the back button on your browser to back out of the survey.   As noted above, you may leave any of the survey questions blank other than the first 13 demographic questions and the conditional response inquiries (i.e., questions 19, 32, and 50 pertaining to the number of visits you have had in the past 12 months to the Student Health Center and the Counseling Center, and the type of health insurance you have).  If you discontinue the survey prior to completing page 11, and later decide to complete the survey, you will have to contact the survey manager to obtain a special six digit access number.

The aggregate survey data will be maintained on this web site after the completion of the survey.  The aggregate survey data, however, will not be made available to any parties that are not approved by the University of Northern Colorado to receive survey data.

At the conclusion of the survey, you will be given the option to provide comments to the consulting firm that is assisting the University with this research effort, Stephen L. Beckley & Associates (SLBA). Providing SLBA with any comments you may have about the survey is completely optional.
