The University of Pennsylvania is conducting a web-based survey pertaining to health care issues for students. This important research effort will help to provide guidance for the future operation of the University of Pennsylvania Student Health Service. If you would like to know more about the Student Health Service before taking the survey, please visit us at www.upenn.edu/shs.

We expect the survey will take you approximately 10 minutes to complete. You may preview the survey by using the pseudo identification number: 123456.  The only inquiries that you are required to answer are demographic questions 1 through 14 pertaining to sex, age, ethnicity, country of citizenship, etc., and question 48 about previous use of the Student Health Service. A response to Question 48 is required because you will be sent to a different set of questions depending whether you have used or not used the Student Health Service. Other than these questions, you may leave all other questions blank.  When you click on the "submit" button at the bottom of each page, the survey software will call your attention to any questions you did not answer.  You may then use the "back" button on your web browser to answer the missed question(s), or click on a "continue" button to bypass the question(s) you choose not to answer.  If you inadvertently answer a question incorrectly, there is a button at the bottom of each page of the survey that allows you to reset all of the answers on that page.

This survey resides on a non-Penn server because an outside consultant is tabulating the results and making an indepdent report to the Student Health Service. The web site used for the survey complies fully with the security requirements established by the University of Pennsylvania (please refer to http://www.upenn.edu/computing/security-privacy/standards/wwwsec.html). The use of your partial Student Identification number (i.e., Social Security number) is only requested to control access to the survey (you may only take the survey one time) and to determine who is eligible for the participation prizes. Please be advised that your total Social Security number has not been provided to any entity outside of the University of Pennsylvania. Under no circumstance is your partial identification number associated with the data string-line for your responses to this survey. While no one can guarantee complete confidentiality for information transmitted via the Internet, we believe that all reasonable steps have been taken to assure the confidentiality of this survey.
