The Colorado School of Mines periodically asks its students to participate in surveys pertaining to its student health program: the Coulter Student Health Center, the Student Health Benefits Plan, and counseling services available through Student Development and Academic Services. Our last survey in 1998 provided important student perspectives that supported our providing dental care services on campus.

You may already be aware of the large cost increases that are occurring in Colorado and nationally for health insurance programs. While our student health insurance program provides an outstanding value, we are not immune to the adverse cost trends that face health insurance program managers. Even if you are covered by personal health insurance, or you have not yet used Coulter Student Health Center, your participation in this survey is important. It will help us to make program management decisions for the coming year.

You may only complete the survey one time. If you cannot access the survey using the last six digits of your Student Identification number, please contact the consultant for CSM, Stephen L. Beckley, at: beckley@slba.com.

Following completion of your survey, your name will be entered into a drawing to win a $100 gift certificate at the CSM bookstore. We expect the survey will take you less than 10 minutes to complete. The survey includes questions about your use of health care providers and any ongoing health care conditions requiring periodic treatment. You may preview the survey by using the Student Identification number: 123456.

The only inquiries that you are required to answer are the demographic questions 1 through 14 pertaining to sex, age, ethnicity, country of citizenship, etc., and question 23 ("How often have you visited the Coulter Student Health Center in the past 12 months). You may leave any other question blank. When you click on the "submit" button at the bottom of each page, however, the survey software will call your attention to any questions you did not answer. You may then use the "back" button on your web browser to answer the question, or click on a "continue" button to bypass the question(s) you choose not to answer. If you inadvertently answer a question incorrectly, there is a button at the bottom of each page of the survey that allows you to clear all answers on that page (or you can simply hit the refresh button on your browser to reset the page).
