Thank you for visiting our student survey pertaining to health care costs, insurance benefits, and the services provided by the Student Care Center (SCC). The deadline for participation in the survey is Friday, November 18, at 5:00 PM.

As a special incentive, all students who enter their unique access code into the survey site will be eligible for a drawing for the prizes mentioned in the survey announcement. This access code will assure your survey responses are kept confidential. Your personal identity will not be linked to your responses. You can log in only once to the survey site using your access code. However, you can preview the survey before logging in by using the preview code of 123456.

Survey respondents are required to answer the first 11 questions pertaining to demographic information, as well as several conditional response questions. How you answer the conditional response questions determines which set of questions will follow. Most of the survey questions are optional.

Before you log-in, I suggest you obtain a copy of your current health insurance card or policy. Although it is not required that you have a copy of your card/policy in hand, it may be helpful as you answer several questions on the survey. Also, think about your health care expenses over the past year; the survey will ask you to estimate how much you have spent out-of-pocket for your health care and prescription needs.
