Welcome to the national survey of student health insurance programs provided by private colleges and universities. This survey is sponsored by Dartmouth College and Stephen L. Beckley & Associates, Inc. (SLBA), of Denver, Colorado.

You may gain access to the survey by using the postal zip code for your institution. We expect the survey will take you approximately 20 to 30 minutes to complete. You are welcome to preview the survey by using the code: 12345. You may also find it helpful to print from your web browser software some of the survey questions that are predicated on detailed definitions (e.g., question four pertaining to student health insurance enrollment system definitions).

There are 27 questions in this survey. The only inquiries that you are required to answer are our demographic questions 1 through 13 (e.g., location of your institution, size of student population, type of student health insurance enrollment system, insurance plan funding arrangement, etc.) You may leave blank questions 14 through 27. When you click on the "submit" button at the bottom of each page, the survey software will call your attention to any questions you did not answer. You may then use the "back" button on your web browser to answer the question, or click on a "continue" button to bypass the question(s) you choose not to answer. If you inadvertently answer a question incorrectly, there is a button at the bottom of each page of the survey that allows you to clear all answers on that page.

Please note that we are not providing a promise of confidentiality for data submitted in response to this survey. The data analysis website will not identify colleges and universities on an individual-institution basis. Reports from SLBA and Dartmouth college following the survey will also not identify data provided by individual institutions.

At the conclusion of the survey, you will be given the option to send email to Stephen L. Beckley & Associates providing comments or additional information requested in the survey. Providing SLBA with your name, telephone number, email address, and any comments you may have about the survey is completely optional.
