Health Care Survey
THIS SURVEY IS NOW CLOSED -- YOU
MAY CONTINUE TO PREVIEW THE SURVEY, BUT NO DATA
WILL BE WRITTEN FOR YOUR RESPONES
Greetings from W. Hal Godwin
Vice President for Student Affairs and Vice Provost
The University of Idaho is conducting an extensive study of health care
and insurance issues among our students. The deadline for participation
in the survey is midnight on Wednesday, May 10,
2000.
TRAVEL, EAT, STUDY! To
show our appreciation for your response to this survey, we will have a
drawing for one $350 gift certificate from Departure's Travel for students
who complete the survey by midnight on Saturday, April 29.
Other survey participation incentives will be announced soon.
We expect the survey will take you approximately 10 to 15 minutes to
complete. The survey includes questions about your use of health care providers
and recent health care conditions that required treatment. You may preview
the survey by using the Student Identification number: 123456. The
only inquiries that you are required to answer are as follows:
Demographic questions 1 through 13 pertaining to sex, age,
ethnicity, country of citizenship, etc.,
Questions 18 and 29 (number of visits to the Student Health Service
and number of sessions at the Student Counseling Center)
Question 45 ("What is the source of your health insurance coverage?").
Questions 18, 29, and 45 are required because the set of questions you
will receive following each inquiry will be different depending upon your
answer (e.g., you will be sent to different questions if you have zero
visits to the Student Health Service in the past 12 months versus an answer
indicating you had one or more visits to the Student Health Service).
Other than the questions noted above, you may leave all other question
blank. When you click on the "submit" button at the bottom of each
page, the survey software will call your attention to any questions you
did not answer. You may then use the "back" button on your web browser
to answer the question, or click on a "continue" button to bypass the question(s)
you choose not to answer. If you inadvertently answer a question
incorrectly, there is a button at the bottom of each page of the survey
that allows you to clear all answers on that page.
We are doing everything possible to assure the confidentiality of the
survey participants' responses. For example, the partial use of your
Student Identification number is only complete the drawing for selection
of award recipients. Although we have taken appropriate steps relative
to survey data confidentiality, such as using a secure server, there is
no way to provide an absolute assurance that data transmitted over the
Internet will remain secure.
At the conclusion of the survey, you will be given the option to provide
comments to the consulting firm that is assisting the University with this
research effort, Stephen L. Beckley & Associates (SLBA). Providing
SLBA with any comments you may have about the survey is completely optional.
Please contact our survey manager, Tonya
Snyder, if you have questions about the survey or have difficulty accessing
the survey using the last six digits of your student identifcation number.
Thank you for participating in this important research effort about
the future of counseling, health care services, and health insurance programming
at the University of Idaho. Best wishes for good health and success in
your academic endeavors at the University of Idaho..
