The University of Michigan is conducting an extensive study of health care and insurance issues among our students. You have been randomly selected to participate in this survey.  This research effort will help us to evaluate how health care services at the University Health Service for students should be funded, and the role (if any) the University should take in providing health insurance coverage for students.

You may only access the survey one time.   The deadline for participation in the survey is midnight on Tuesday, April 18, 2000 Friday, April 21, 2000.

TRAVEL, EAT, STUDY! To show our appreciation for your response to this survey, we will be awarding a $300 Travel Voucher on Northwest Airlines to one lucky respondent.  We will also be awarding a $100 gift certificate to the Gandy Dancer and a $25 gift certificate to Michigan Book & Supply.  The random drawing of winners will take place on May 1.

We expect the survey will take you approximately 10 to 15 minutes to complete. The survey includes questions about your use of health care providers and recent health care conditions that required treatment. You may preview the survey by using the Student Identification number: 123456. The only inquiries that you are required to answer are the demographic questions 1 through 13 pertaining to sex, age, ethnicity, country of citizenship, etc., and questions 19 (number of visits to University Health Service in past 12 months) and 33 ("What is the source of your health insurance coverage?"). You may leave any other question blank. When you click on the "submit" button at the bottom of each page, the survey software will call your attention to any questions you did not answer. You may then use the "back" button on your web browser to answer the question, or click on a "continue" button to bypass the question(s) you choose not to answer. If you inadvertently answer a question incorrectly, there is a button at the bottom of each page of the survey that allows you to clear all answers on that page. 

We are doing everything possible to assure the confidentiality of the survey participants' responses. For example, the partial use of your Student Identification number is only used to control access to the survey and complete the drawing for selection of award recipients. Although we have taken appropriate steps relative to survey data confidentiality, such as using a secure server, there is no way to provide an absolute assurance that data transmitted over the Internet will remain secure. 

At the conclusion of the survey, you will be given the option to provide comments to the consulting firm that is assisting the University with this research effort, Stephen L. Beckley & Associates (SLBA). Providing SLBA with any comments you may have about the survey is completely optional. 

The survey includes questions about health insurance coverage and the use of health care services. Questions regarding health insurance may cause anxiety or other concerns if you do not presently have health insurance coverage and questions regarding use of health care services may trouble persons currently using health care services.  If you are anxious or troubled by any such questions, you may disregard this survey or merely leave these questions blank.  All survey responses are provided anonymously and individual responses are kept confidential. 

If you have questions about the survey, you may contact the Managed Care Department at the University Health Service by telephone at (734) 764-5182. Our survey has been approved by the University's Institutional Review Board. This Board oversees the use of research projects involving human subjects.
