Parent/Guardian Survey
The Survey is now closed.
The University of Michigan is conducting an extensive study of health
care and insurance issues among our students. As a parent/guardian of a
student presently enrolled at the University, you have been randomly selected
to participate in this survey. This research effort will help us
to evaluate how health care services at the University Health Service for
students should be funded, and the role (if any) the University should
take in providing health insurance coverage for students.
If you have questions about the survey, please contact Nancy Helmick
at the email or phone numbers listed below. The deadline for participation
in the survey is July 5, 2000.
We expect the survey will take you approximately 10 minutes to complete.
We are doing everything possible to assure the confidentiality of the survey
participants' responses. Although we have taken appropriate steps
relative to survey data confidentiality, such as using a secure server,
there is no way to provide an absolute assurance that data transmitted
over the Internet will remain secure.
At the conclusion of the survey, you will be given the option to provide
comments to the consulting firm that is assisting the University with this
research effort, Stephen L. Beckley & Associates (SLBA). Providing
SLBA with any comments you may have about the survey is completely optional.

approved by the University's Institutional Review Board. This Board oversees
the use of research projects involving human subjects.
