The Ohio State University Wilce Student Health Center & Insurance Survey
The Wilce Student Health Center is sponsoring this survey on various health
and insurance issues among OSU students. Approximately 3,000 students during
the 1997-98 summer session have been randomly selected to participate in this
survey. You may only complete the survey one time and you must be among the
students randomly selected to access the survey (using your Student
Identification number). Following completion of your survey, you will be
entered in a drawing to win $50 on your I.D. card. One $50 award will be
made for every 500 survey participants.
We expect the survey will take you approximately 10 minutes to complete.
Questions 13, 14, 15, 17, 18, 19, and 20 are only being asked of survey
respondents who indicate they are presently covered by the OSU student health
insurance plan on question 11. If you answer that you have some other form
of health insurance coverage, or that you are uninsured, you will not be
shown these questions.
Questions 1 through 11 must be answered for you to participate in this
survey.

Please note that your response to this survey will be kept confidential and
will not be associated with your Student Identification number. We expect to
post summary results of the survey on a home page later this summer. We hope
that you enjoy your experience with this survey using the World-Wide-Web.
Note: The survey is now closed, but you can proceed
and see what the survey was all about. No further data will be added to
the data base if you do take the survey as an experiment.
