University of Massachusetts Amherst Health Care Survey

SURVEY INSTRUCTIONS

University Health Services is conducting an extensive study of health care and insurance issues for UMass students.

After you finish reading this page, click on the "I'm willing to participate in the survey" button to access the survey. The next page is the "Survey Entry Form." Type the last six digits of your Social Security number in the box. If you are an international student, please use the number that the University assigned for you to use when you are asked for a Social Security number. (It looks something like this: 999-34-5678.) You will type in the six digits after 999. If you cannot access the survey using the last six digits of your Social Security number, please contact Rob Morgan at University Health Services (rmorgan@uhs.umass.edu)

The rest of the survey is self-explanatory. (You will not need to remember these instructions.) When you click on the "submit" button at the bottom of each page, the survey software will call your attention to any questions you did not answer. You may then use the "back" button on your web browser to answer the question. If you inadvertently answer a question incorrectly, there is a button at the bottom of each page of the survey that allows you to clear all answers on that page.

Your responses to this survey are completely confidential. The partial use of your Social Security number will not be associated in any data file with your survey answers; we will use it only to track who has completed a survey. Also, the survey is operated on a secure server.
