The Graduate Assistant Plan 

The Araz Group is the PPO for the Graduate Assistant plan.  Graduate Assistants covered by the plan may select their own health care providers within the Araz network.  The plan provides 100-percent coverage, except there are co-payments for prescription medications ($8 per prescription or refill), chiropractic care, physical therapy, and urgent care/emergency room visits.  The plan includes coverage for out-of-network/out-of-area emergency care services.  There is a $1 million lifetime maximum benefit under this PPO coverage, and there is no pre-existing condition exclusion.  Coverage may be extended for up to 18 months after termination of a Graduate Assistant appointment (premiums are paid by the student during that period).  The plan also offers 3 levels of optional coverage for the spouse/same-sex domestic partner or child(ren) of Graduate Assistants.  The University pays 55% of lowest-cost option; GA pays the remainder of premium.  Under a separate contract, GAs also have coverage for preventive dental care services and deeply discounted restorative care at the U of MN Hospital Dental Clinic; the same deep discounts also are available for all GA dependents.

The University of Minnesota's Student Health Insurance Plan 

All degree-seeking students who pay the student services fee or extended coverage fee and enroll for six or more daytime or extension credits (or a combination of both) are eligible to participate in the University of Minnesota Student Health Insurance  Plan.  The plan is underwritten by Reliance National Insurance Company.  The spouse/same-sex domestic partner or child(ren) of a covered student also are eligible for coverage.  The cost per semester for the 1999-2000 plan year is (the spring semester automatically includes coverage for the summer session):

  • Tier One - Boynton Health Service (Covered Students Only) 100 percent coverage (without a pre-existing condition exclusion). There is a $12 copay for prescription medications ($750 annual maximum benefit), and there is a $10 copayment for mental health care services ($250 maximum per plan year) 
  • Tier Two - PreferredOne Provider Network The plan pays 80 percent of the first $10,000 of eligible expenses for care rendered by PreferredOne providers (http://www.preferredone.com), and 100% thereafter.  Outpatient prescription drugs are excluded from both Tier Two and Tier Three. 
  • Tier Three - Other Providers The plan pays 70 percent of charges subject to a one-time annual deductible of $200. The combined maximum for copayments, deductibles, and coinsurance is $2,000 in any plan year. The lifetime maximum benefit is $500,000.

    Pre-existing conditions under Tier Two or Tier Three are not covered until the student has been insured by the plan for 12 months.
