2003 National Student Health Program SurveyDear Student: A randomly selected population of students from each of the public universities listed on the left is being invited to participate in a national survey pertaining to health care and insurance concerns. This research effort will allow your university to obtain important benchmark information from its peer institutions. Previous survey participants have included Ohio State University, the University of California at Berkeley, and Colorado State University. You may only access the survey one time. If you received a letter or email message inviting you to participate in the survey, and you cannot access the survey using the last six digits of your Student Identification number (for University of Idaho students, your access ID number was provided in the email message you received), please click on the "Questions" for your university on the left to send email to the survey manager. We expect the survey will take you less than 10 minutes to complete. The survey includes questions about your use of health care providers and recent health care conditions that required treatment. You may preview the survey by using the pseudo student identification number of 123456. The only inquiries that you are required to answer are the demographic questions 1 through 14 pertaining to sex, age, ethnicity, country of citizenship, etc., and question 23 (number of visits to the student health service in the past 12 months) and question 50 ("What is the source of your health insurance coverage?"). You may leave any other question blank. When you click on the "submit" button at the bottom of each page, the survey program will call your attention to any questions you did not answer. You may then use the "back" button on your web browser to answer the question, or click on a "continue" button to bypass the question(s) you choose not to answer. If you inadvertently answer a question incorrectly, there is a button at the bottom of each page of the survey that allows you to clear all answers on that page. We are doing everything possible to assure the confidentiality of the survey participants' responses. For example, the partial use of your student identification number is only used to control access to the survey and for the prize drawing. Although we have taken appropriate steps relative to survey data confidentiality, such as using a secure server, there is no way to provide an absolute assurance that data transmitted over the Internet will remain secure. The survey includes questions about health insurance coverage and the use of health care services. Questions regarding health insurance may cause anxiety or other concerns if you do not presently have health insurance coverage and questions regarding use of health care services may trouble persons currently using health care services. If you are anxious or troubled by any such questions, you may disregard this survey or merely leave these questions blank. All survey responses are provided anonymously and individual responses are kept confidential. This survey may have been approved by the institutional review board at your university that oversees research projects that involve human subjects.
